It is considered that you have agreed to these terms at the time of making a reservation at our shop or starting this service.

Article 1 Definition

1. 1. Shiatsu / Massage Mikore Yoyogi (hereinafter referred to as "our shop") provides users with services using shiatsu / massage / acupuncture (hereinafter referred to as "this service").

2. Our shop is provided by the holders of national qualifications "Anma Massage Acupuncturist" and "Acupuncturist" for the purpose of improving the condition and health of users.

3. 3. Our shop may not be able to treat acute diseases, severe hernias, etc. Please note.

4. This agreement, service content and terms of use can be changed at any time at our own discretion. The notification can be made by posting in the store, communicating by our staff, or posting on the homepage, and it shall take effect at the time of notification.

 

Article 2 application

This agreement shall be applied to all relationships related to the use of this service between the user and our shop, and shall be carried out based on this agreement.

 

Article 3 User registration

1. 1. Before using for the first time, please provide your name, address, phone number and emergency contact information.

2. We will manage the information you provide appropriately and use it for our reception work, management work, contact work, etc. We will not divert anything unrelated to this service of our shop.

3. 3. Due to the need for this service, registration of false personal information is strictly prohibited.

4. If there is a change in the registered contents, you will be required to declare the changed contents when you visit the store for the first time after the change.

 

Article 4 Reservation / Change / Payment

1. 1. Our shop can be used only by users who have made a reservation in advance by WEB, telephone, or visit.

2. Our shop is not in charge.

3. 3. Please note that a separate nomination fee will be charged when appointing a person in charge. In addition, only if the user is a woman, we will accept "female staff request" free of charge.

4. You can use it within the reserved time. However, it may be extended as necessary at the discretion of the staff.

5. The length of usage time depends largely on the user's condition, so it may differ each time, and there may be slight differences depending on the user and staff. Please note.

6. Please be sure to contact us in advance if you wish to cancel or change your reservation. Please note that you may be charged a cancellation fee if you cancel without prior notice.

7. Please note that the person in charge will contact you if you do not come to the store even after the reservation time has passed.

8. Please note that if you arrive after the reservation time, we may not be able to accept your request on the day of the event.

9. Please note that you will be charged the normal fee if the service is terminated due to the user's convenience.

 

Article 5 Confidentiality

1. The staff of our shop shall be obliged to keep confidential the secrets (important privacy-related conversation contents and information about illness) that we asked the user. However, this does not apply in the following cases.

① When there is an order from the judiciary or an administrative agency.

② When the consent of the person is obtained.

③ When it is judged that the information is known to the person to be disclosed before disclosure from our shop.

④ When it is judged that it is necessary to disclose to the partner (doctor, family, etc.) for the protection of the person.

⑤ When it is judged that there is a possibility of causing serious harm to the person or others.

2. Please note that other staff members may share the information obtained through prior interviews before using the store, as our shop is not in charge.

 

Article 6 Prohibitions

The following acts are prohibited, and our shop has the authority to refuse the use of services of those who fall under the following.

1. 1. Acts that cause harm to our staff. (Physical harm, sexual harassment, ranting, etc.)

2. Annoying acts to other users.

3. 3. False declaration of usage registration information, and no declaration of illness, trauma, chronic illness, or important medical history.

4. Actions and conditions that worsen the hygiene of our shop.

5. Use that does not meet the purpose of our shop.

6. Make unreasonable demands that exceed the situation and abilities of our shop and staff.

7. Annoying acts to our staff outside the store.

8. Dissemination of rumors that differ from the facts about our shop on the Internet, various media, orally, and dissemination of personal information of our staff.

9. Other acts that violate laws and regulations or public order and morals.

 

Article 7 Disapproval of use

In the following cases, we may not consent to your use or may limit your use.

1. 1. If you have a disease or disability that is beyond the scope of our service.

2. When repeated acts that interfere with our business, such as cancellation without permission.

3. 3. When a person different from the one at the time of reservation visits the hospital.

4. When communication with staff is difficult.

5. In addition, when the person in charge of our shop or the staff judges that it is difficult to continue.

 

Article 8 Disclaimer

1. 1. If you do not report incorrect information or inform us of matters to be considered in advance, we will not be liable for any disadvantages caused by this.

2. Our shop is not intended for treatment of illness.

3. 3. All the staff will make the utmost efforts in good faith, such as maintaining health and alleviating symptoms. However, the effect cannot be guaranteed.

4. If it is judged that it is absolutely necessary for the safety of the person or our shop, and there is no other means, contact / consultation with the government (emergency / police and other organizations), medical welfare institutions, and the person's family at the discretion of our staff. However, we may ask for advice and protection.

5. Although we give maximum consideration to the usage environment, please understand that there are limits to temperature, humidity, noise, and privacy protection due to the location and property facilities.

6. In the event of unavoidable reasons (disaster, stormy weather, sudden illness of staff, traffic obstruction, in-store system malfunction, unavoidable mistakes, other situations where service cannot be provided, etc.), we can cancel or change your reservation. Suppose. In that case, we will contact you as much as possible, so please be sure to provide the contact information that you can contact.

 

Article 9 Court of Jurisdiction

If there is a need for a proceeding between the user and our shop, the court of jurisdiction where our shop is located shall be the exclusive agreement jurisdiction court of the first instance.
